In order to guarantee your patients receive the assistance they need without delay, the first step must be a doctor referral.

The services we offer to patients if eligibility requirements are met are:

Transportation to and from treatment or follow-up doctor visits related to cancer-care
Childcare during treatment
Education and support meetings
Cosmetic aide and hygiene products
Respite care services
Temporary housing due to geographical distance from the treatment center
Food during treatment
Medical supplies and equipment
Cancer screenings
Any other supplies or services related to cancer treatment

To be eligible for a grant, the grantee must meet all the following requirements:

Must have a social security number
Must be working, disabled, a student, or retired
Must be referred by a qualified physician
Must be a resident of the United States
Must be currently under treatment for cancer, be within 6 months of a cancer diagnosis, have completed treatment within 6 months, be in need of cancer screening and in need of financial assistance

Grants will be made based on an application that can be submitted at any time to the main office of 21st Century C.A.R.E. by fax, e-mail, or regular mail. Include a copy of patient’s social security card and a list of any other community sources being utilized for the same purpose.

Once all the requested information is received, the application will be processed quickly so the patients receive the assistance they need without delay. Grants are made on a case-by-case basis by 21st Century C.A.R.E.’s Financial Assistance Committee. Grants will be awarded without regard to race, national origin, gender, or sexual orientation. Grants will not be made for co-pays, deductibles, prescription medication, household bills, utility payments, rent, mortgage, etc. at this time. The Committee reserves the right to limit, suspend or terminate this financial assistance program if sufficient funds are not available. All information received will remain confidential.
